Participation in Formation Haus classes may require completion of a separate liability waiver.

Please include sufficient information for us to identify and verify your account, such as the name and email address associated with your Formation Haus account.
After verifying the request, Formation Haus will process the deletion of personal information associated with the account, subject to information that we or our service providers are required or permitted to retain for legal, tax, accounting, security, fraud-prevention, dispute-resolution, or other legitimate purposes.
